What is a Connection?
A "Connection" occurs when we have our first face to face meeting with an adult who has a child onset intellectual or developmental disability diagnosis, and is also without a Medicaid Waiver resource. The purpose of the connection is to confirm or establish the appropriate CCB and to confirm his or her place on the Medicaid Waiver waiting list.
During this first meeting, we ask a series of twenty-five general questions to establish a level of need and determine appropriate connections to other community resources and natural supports.
